Colchester Medical Practice has an opportunity for a care coordinator to join our team on a full or part time basis, with working hours ranging between 25-37 hours, preferably across five days a week. This will be based at our Wimpole Road Site, working within our proactive care team. The post-holder will provide extra time, capacity and expertise to our practice team to help us to improve the car
