Liquid Personnel is recruiting an Adult Social Worker to join its client’s Preparation for Adulthood team, based in Sheffield. What will your responsibilities be? Transition Planning — Lead holistic, person‑centred transition assessments for young people (14–25) moving from children’s to adults’ services, ensuring Care Act eligibility, strengths‑based planning, and coordinated multi‑agency pathway
